IMPORTANT INSTRUCTIONS

To complete the required PHS online you must click on the link below to begin. Once you click on the link you will be directed to the Personal History Statement (PHS). You will have 24 hours to complete the PHS before the time to complete it resets and you will lose all the information you input.  

Things to remember

• Click the "ADD" button after entering information to be saved
• Click "SAVE CURRENT SECTION" before proceeding to the next page
• When you have completed filling out the PHS click generate PDF, which will will give you a button "GENERATED PDF". click on the "GENERATED PDF" button and save the pdf with your name
• Upload the completed PDF using the "UPLOAD PDF" button on this websites main page.
