Download the required Personal History Statement to begin your background investigation process. Once you have downloaded the PDF, select the "Open in Desktop App" located in the top right corner of the PDF , rename and save to your desktop. You must have Adobe Reader to complete the PHS. You may download a free version of Adobe Reader by clicking on the following link https://get.adobe.com/reader/?promoid=TTGWL47M .
DO NOT print off and fill in PHS by hand. All listed family, personal and work references must have valid email addresses listed in the PHS.
You will only need to submit the PHS and required agency waiver to being the background process.
